The Momentum Project is a VillageCare community AIDS program

 

VillageCare is a community-based, not-for-profit service organization serving the older adults, persons living with HIV/AIDS and individuals in need of medical and rehabilitation services. Guided by the people we serve, we provide leadership to improve the health and quality of life of the diverse communities we serve and the quality of care and well-being of the people we serve.

 

For the person living with HIV, the Network of AIDS Services offers unique, and complete care. Whatever the stage of the disease, the Network has a care offering to match each individual’s needs. And as those needs change, persons in our care move seamlessly into the appropriate Network offering, receiving the right level and intensity of care in the setting that is best for them.

 

When the AIDS epidemic first struck, VillageCare pioneered services that made sense for those with HIV infection - innovative programs that treat people with respect and which recognize that those living with this disease must be involved in decision-making. We understand better than anyone how important it is for those who are HIV-positive to have a broad and interrelated set of medical and psycho-social care offerings and services.

 

Today, VillageCare’s Network of AIDS Services has two adult day treatment centers, a certified home health agency and community case management, The Momentum Project along with Rivington House, a state-of-the-art residential health care facility.
