Responsibilities
-
Responsible
for the coordination, monitoring and evaluation of all operations to
provide foods and supplies necessary for best possible use of the food
programs and other supportive services.

Responsible
for liaison with linkages, with government food programs, as well as
with donating individuals and organizations. To ensure smooth
operations.
-
Responsible
for providing sites with daily paperwork needed. Examples include but
not limited to DOH agreements, cleanup sheets, weekly menus, monthly
calendars, etc.

Coordinate
with the Director of Food/ Site Services to develop annual plan.
-
Coordinates
with Nutritionist, and Executive Chef in all appropriate areas of Food/
Site operations.

To work with
site registration as needed
-
Responsible
for the proper training, evaluation and supervision of all Site
Coordinators.
-
Responsible
for the daily as well as vacation coverage of all Site Coordinators.
-
Responsible
for the prompt delivery of paperwork included, but not limited to
inventories, stock usage sheets, and invoices.
-
Coordinate with the Dir. of Food/Site Services to analyze and implement
new systems as related to Food Services and Site Operations.
Qualifications
BS with minimum of 5 years experience in
non-profit administration supervisory skills, organizational skills and
time-management skills.
Flexibility. Some knowledge of
HIV/AIDS issues. Familiarity
with purchasing procedures. Computer skills and
willingness to learn. Dedication to mission and ability to adapt.

COOK/CHEF
RESPONSIBILITIES:
The
responsibility of the Chef is to prepare site meals in cooperation with the
Director of Food/Site Services. The Chef is reports to the Dir. of
Food/Site Services.
-
Execute detailed assigned menus, utilizing provided ingredients in
accordance with direction from the Director of Food Service and in
compliance with all New York City Department of Health regulations.
-
Prepare and supervise all significant primary recipe ingredients
including, but not limited to: washing, coring, paring, and slicing of
ingredients.
-
Enforce the basic principles of good nutrition and food
safety/sanitization as related to specific site operations.
-
Monitor site inventory and advise supervisor of inventory status
including need for supplies to operate and maintain day-to-day kitchen
operations.
-
Coordinate with site coordinators for timely and appropriate meal
delivery.
-
Supervising kitchen volunteers during site operations in preparation and
delivery of meals.
-
Act
as a representative of Momentum in the community to create positive
visibility and promote the support of others.
-
Contribute to interdepartmental collaboration to enhance overall program
effectiveness.
QUALIFICATIONS:
High
School graduate. Minimum two years in food preparation and cooking.
Must have the ability to work independently and under time-constraint
pressures. Good supervisory skills. Flexible. Familiarity
with HIV/AIDS nutrition issues a plus. New York State Food Handlers
Permit is required. Computer literate.

Nurse (Consultant)
Responsibilities:
The position
provides individual and group counseling & education to clients regarding
HIV-related problems & other physical/psychosocial responses to HIV disease
and its treatment. The
Consultant Nurse reports to the Nursing/Health Education Coordinator/RN
Supervisor.

Through
outreach, referral & counseling, evaluate individual client’s perceived
health status and status changes.

Educate
clients in order to assist clients to develop and implement
health-promoting & effective coping behavior.
-
Assist
clients with self-assessment of health status in order to provide
clients with information about treatments available & how to access
them.
-
Promote
continuity of care by facilitating access to other community health-care
resources.
-
Support
staff efforts to promote primary prevention.
-
Provide
periodic updates to staff regarding advances in knowledge about HIV
pathologies & treatments.
-
Keep current
with client contact sheets/charting, time sheets.
-
Participate
in “Quality Assurance.” / monthly supervision
Qualifications:
RN plus minimum of three years experience in HIV/AIDS services.
Bi-lingual a plus.
Ability to write and communicate effectively.
Familiarity & skill in working with diverse communities within a
multi-disciplinary team. Must have personal malpractice insurance policy.

pastoral Care Consultants
Responsibilities:
The primary
responsibility of the Consultant/intern is to deliver spiritual care to
people with HIV/AIDS, who are clients of the Momentum Project, Inc.
The Pastoral Care consultants report to the Coordinator of SW
Services.

To
service clients at assigned sites, providing active listening and
spiritual assistance with death/dying and grief/loss issues.
-
To
provide presence/reflection during circle gatherings, consistent with
agency Mission Statement and department guidelines.
-
To
facilitate support groups where appropriate.
-
To
work as a member of the Client Services interdisciplinary team and with
other Momentum staff (paid and volunteer), making and receiving
referrals and consulting as appropriate to ensure coordinated and
quality care for clients.

Participate in scheduled supervision for purposes of communication,
professional support, accountability and increased department/agency
effectiveness.

To submit to the
Coordinator
of SW Services accurate weekly
Contact/PIE sheets on encounters with clients, documenting levels of
service. Submit biweekly time sheets to Team Leader.

display professional standards/behavior that enforces and upholds the

Qualifications:
M. Div. or
equivalent, HIV/AIDS experience.
For students, at least one completed year of seminary.
Preferably, student will have completed one unit of CPE and/or a
supervised ministry in HIV/AIDS.

HOUSING ADVOCATE
Responsibilities
-
Assesses both new and old clients’ financial eligibility to determine
what public benefits and entitlements are needed to sustain highest
quality of life.
-
Assesses housing status on all clients and link clients to various
housing services as appropriate.
-
Analyzes appropriate benefits for clients’ needs when assisting clients
in completing benefit applications to correct knowledge deficits and
make appropriate referrals when necessary.

Develops and facilitates group training programs for clients around
entitlement housing, immigration and health benefits issues.

Participate in intake and orientation activities for new clients.
-
To
attend assigned weekly or weekend sites for time determined in
consultation with the Director of Client Services or the Coordinator of
Social Work Services.

Acts
as a liaison with other AIDS funded organizations to create and enhance
cooperative relationships and to foster sharing of resources.

Attends and participates in staff meeting and assists in planning for
such to address program related issues and to share information of
resources.

Provides weekly client contact information for accurate reporting in
agency database.
Qualifications
BA degree or
equivalent combination of college and experience. Comprehensive knowledge of
all public entitlement programs; administrative experience; Bilingual
(Spanish) helpful; Ability to write and communicate effectively. Familiarity
and skill in working with divers communities a multi-disciplinary team.
